As a provider of Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services, Advanta Partners operates as a Business Associate (BA) to your healthcare provider (the Covered Entity). Our handling of all Protected Health Information (PHI) is strictly governed by the Business Associate Agreement (BAA) and all applicable HIPAA regulations.
Key Principles for PHI

Purpose of Use: We only use and disclose PHI as necessary to carry out the functions of treatment, payment, and healthcare operations as mandated by the BAA and your provider. This includes coordinating care, providing patient support, and documenting services for billing.

Patient Rights: Your rights regarding access, amendment, or restriction of your PHI are governed by the Notice of Privacy Practices (NPP) provided by your treating physician/clinic. All inquiries regarding your medical records must be directed to them.

2. Information We Collect
We collect two primary categories of information:
A. Protected Health Information (PHI)
This information is collected only through our service platform on behalf of the partnering medical practice and includes:

B. Personal Information (PI) & Website Data (Non-PHI)
This information is collected via the website from prospective clients, partners, and job applicants:
