Proactive Care at Work: What Our Latest Data Tells Us About Preventing Hospitalizations

At Advanta, we believe that the best kind of healthcare is the kind that keeps patients out of the hospital. Over the past several months, we’ve been diving deeper into our Chronic Care Management (CCM) escalation data - and the early results have confirmed something we’ve always believed: when you meet patients where they are, consistently and compassionately, you can change the trajectory of their care.


From February to April 2025, we tracked 88 patient escalations across a population of 5,000 enrolled in our CCM program. These escalations represent critical moments when patients needed clinical attention - whether they were experiencing new symptoms, worsening chronic conditions, or medication side effects. What stood out to us was this: 92% of patients who followed our escalation pathway did not require an ER visit, and 100% avoided hospitalization beyond the ER. That means our team successfully intervened - through nurse-led assessments, medication counseling, patient education, or provider coordination - before things turned critical. Most notably, two escalation types dominated our dataset: Newly Reported Symptoms (25%) and Exacerbation of Chronic Conditions (20.5%).


These are the kinds of issues that, without timely attention, often lead straight to urgent care or hospital admission. Yet our care model, built around regular check-ins, dedicated care managers, and Registered Nurse oversight, was able to catch these issues early and resolve them with non-emergent interventions. We don’t take these numbers lightly. Behind every ER visit prevention is the story of real a person—a senior with diabetes who avoided a dangerous infection, a patient with COPD who stayed out of the ER during a flare-up, or someone newly diagnosed with a heart condition who got immediate answers instead of panic. It’s a powerful reminder of the human impact of consistent, proactive care.


This was only a two-month snapshot, but it’s already revealing the long-term potential of our model. As we expand, we’re committed to even deeper outcome tracking, even more robust triaging protocols for high-risk symptoms, and even better support for our care teams handling complex cases. We know there’s more work to be done, but we’re proud of the foundation we’ve built—and the lives we’ve helped stabilize.

These data are more than just figures on a spreadsheet; they are proof that our shared commitment to early intervention, accessibility, and education is making a real difference in the quality of care for patients around the country.
